Privacy Policy & HIPAA Notice.
How we handle your information, and the rights you have over your health records.
Last updated: October 15, 2026
This notice describes how Colorado Hormones (MNP Hormones PLLC) handles your information, including health information, and your rights. It applies to this website and to our telehealth care.
Information we collect
- On this website: if you join our Medicare waitlist or email us, we receive the contact details you send. If you book a welcome call, scheduling is handled by Google Calendar, which receives the details you enter there.
- When you become a patient: intake and health questionnaires, medical and medication history, lab results, visit notes, prescriptions, messages with your care team, and payment details.
- Partner offers: if you arrive through a partner page, we know which partner sent you. For some offers we ask for a payment card on file before your labs. It is held with our payment processor, and the terms are in our Terms of Service.
- Technical data: our hosting provider, Netlify, and Google Fonts may receive basic technical data such as your IP address and browser type when pages load. We do not run advertising or analytics trackers on this website.
How we use it
- To provide treatment, review labs, prescribe, and coordinate with pharmacies and labs.
- To schedule visits, bill, and respond to your questions.
- To meet legal and licensing requirements.

We use and share your health information for treatment, payment, and healthcare operations, with the labs and compounding pharmacies involved in your care, and as required by law. We do not sell your information and do not use it for marketing without your written authorization.
